Karen,
We were given the copy of the test for the owner and for the copy the owner sends into the AKC. Along with that we were given a flyer for The ID Store that makes them to send away for them. There are three different tags. Two metal large round or small round and a 3 ply plastic round. I couldn't find a web address on the paper but I did a search on the net and found the address here it is

I'll get a picture with Dakota with the certificate later. I'm going to send off for her tag now. I should have done it in Michigan I wouldn't have been charged the 8% sales tax.
